Leave of Absence and Uniform order Forms:
Holiday requests outside of the normal school holidays need to be authorised by the Headteacher. Parents wishing to take their child out of school need to fill out a Leave of Absence Request Form (click on the link below, or ask the office staff for a form). Forms need to be filled out and submitted preferably a week before the first day of absence to allow time for the form to be processed and approved.
Please note: Absences for appointments such as Hospital or Doctors appointments don’t need to go on this form – please see the office staff for an absence pass and notify the class teacher the day before your child is due to miss school due to an appointment.
